How does the Care Management Program work?

Step 1: Consultation

You and a care manager determine your needs.

Our care manager will provide information for you about:

  • Affordable care options
  • Alternative living places
  • Community resources
  • Legal/financial referrals
  • Relationship changes
  • Emotional support

Step 2: Assessment

Our care manager will assess the following:

  • Your health
  • Your physical capability
  • Your emotional status
  • Your environment
  • Your social support
  • Your financial challenges

Step 3: A Plan for You

The plan may include the following:

  • Skilled care at home
  • Physician’s care
  • Counseling
  • Recreational therapy
  • Transportation
  • Social activities
  • A volunteer to assist you
  • Help paying your bills

Our care manager may help you arrange:

  • In-home services and supports,
  • Government programs
  • Daytime activities
  • Transportation
  • Elder law attorney assistance

Step 4: On-Going Support for Your Needs

  • Our care manager will regularly support you and oversee services.
  • We will re-evaluate your plan as your needs change.
  • We will communicate with your relatives near and far, by request.

All our counselors are licensed professionals with additional expertise in child and family therapy and aging issues.
